Pay Range: $28 - $29.60/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:- Manage a caseload of 50-70 members, focusing on long-term care needs.
- Conduct assessments with members, caregivers, and providers to develop comprehensive care plans.
- Monitor service delivery and follow up through in-person visits and telephonic contact.
- Authorize and coordinate referrals for services, ensuring no gaps in care.
- Collaborate with discharge planners, physicians, and other stakeholders to facilitate effective care transitions.
- Assist members with filing and resolving complaints and appeals.
Key Requirements and Technology Experience:- 2+ years of care management experience, preferably in a field setting.
- Bachelor's degree in a relevant field (e.g., Psychology, Sociology, Healthcare).
- Valid Driver’s License required.
- Experience with electronic medical health records and home health.
- Strong technology skills and ability to manage time effectively.
- Experience in discharge planning.
- Familiarity with TruCare software.
