    Description

    We are currently seeking a dedicated and passionate Clinical Liaison Registered Nurse (RN) to join our team at KeystoneCare. This vital role involves acting as an advocate for patients within an acute care setting until they are safely discharged to their homes. The Clinical Liaison RN is responsible for generating referrals for home health, palliative care, hospice, waiver, and private duty services by building strong relationships with case managers and social workers. In addition to meeting with patients and their families, you will ensure a seamless transition of care throughout the discharge process, guiding them through their options and verifying eligibility for care.

    Primary Job Responsibilities:

    • Serve as the primary clinical representative for KeystoneCare Home Health services within assigned hospitals, skilled nursing facilities, physician offices, and community healthcare organizations.
    • Develop and maintain strong relationships with referral sources to promote home health services and ensure seamless patient transitions to home-based care.
    • Assess potential patients for admission eligibility by reviewing clinical information, physician orders, and discharge plans.
    • Collaborate with hospital case managers, social workers, discharge planners, physicians, and interdisciplinary healthcare teams to coordinate appropriate home health services.
    • Educate patients, families, and healthcare professionals regarding KeystoneCare's home health programs, services, and admission criteria.
    • Facilitate a smooth transition from acute care settings to home by coordinating referrals, obtaining required documentation, and communicating care needs to the home health team.
    • Perform clinical evaluations and make recommendations regarding appropriate levels of care and available community resources.
    • Maintain thorough documentation of referral activities, patient interactions, and business development efforts within designated systems.
    • Monitor referral trends and identify opportunities to expand market presence and strengthen referral relationships.
    • Participate in networking events, community outreach initiatives, and professional meetings to increase awareness of KeystoneCare services.
    • Ensure compliance with all federal, state, Medicare, accreditation, and organizational regulations related to patient admissions and home health services.
    • Collaborate with operations, intake, and clinical teams to support census growth and achieve organizational goals.
    • Respond promptly to referral inquiries and provide exceptional customer service to patients, families, referral sources, and healthcare partners.
    • Maintain knowledge of home health regulations, reimbursement guidelines, industry trends, and best practices.
