We are seeking a driven, professional Healthcare Referral Liaison to build relationships with hospital discharge planners, social workers, case managers, rehabilitation centers, and nursing facilities throughout Collier County.

This role is responsible for generating qualified referrals and scheduling in -home assessments with our Assessment Coordinator.
<\/p>
You will be the face of our 24/7 emergency home care response team.
<\/p>
Conduct daily outreach calls to:

Hospitals
<\/p><\/li>
Rehabilitation centers
<\/p><\/li>
Skilled nursing facilities
<\/p><\/li>
Assisted living communities
<\/p><\/li><\/ul><\/li>
Build long -term professional relationships
<\/p><\/li>
Educate discharge planners on:
<\/p>
24/7 emergency coverage
<\/p><\/li>
Live -in care
<\/p><\/li>
Hospital -to -home transition services
<\/p><\/li><\/ul><\/li>
Identify patients discharging within 24–72 hours
<\/p><\/li>
Schedule in -home assessments immediately
<\/p><\/li>
Follow up on referrals and ensure smooth transitions
<\/p><\/li>
Maintain CRM documentation
<\/p><\/li>
Attend networking events when required
<\/p><\/li><\/ul>
Minimum 20 outreach contacts per day
<\/p><\/li>
5–10 referral conversations daily
<\/p><\/li>
3–5 assessment appointments scheduled weekly
<\/p><\/li>
Relationship -building mindset (not pushy sales)
<\/p><\/li><\/ul>
Experience in healthcare, home care, or medical sales preferred
<\/p><\/li>
Strong communication skills
<\/p><\/li>
Confident speaking with clinical professionals
<\/p><\/li>
Organized and CRM comfortable
<\/p><\/li>
Professional appearance for facility visits
<\/p><\/li>
