CareGivers America - NEPA, a Modivcare Home Care company, is hiring a Full-Time RN Case Manager to provide one-on-one pediatric care in the home setting throughout parts of Luzerne, Wyoming and Sullivan counties.

This is a salaried Monday-Friday role with a company vehicle provided.

Territories to cover: Luzerne, Wyoming, and Sullivan counties

Schedule: Full-Time | 40 hours per week (traveling included) | Monday-Friday

Your Impact as a Pediatric RN Case Manager

  • Complete comprehensive patient assessments, including OASIS data collection, within established timelines
  • Develop and oversee individualized pediatric care plans in collaboration with physicians and families
  • Provide clinical direction and supervision to LPNs and Home Health Aides
  • Ensure documentation accuracy, timeliness, and regulatory compliance
  • Communicate consistently with families and interdisciplinary care teams
  • Participate in quality improvement initiatives and staff education
  • Maintain a safe, respectful, and supportive care environment

Qualifications

  • Active Pennsylvania RN license (Required) - BSN preferred
  • 1 year of professional nursing experience preferred
  • Pediatric or home health experience preferred
  • Strong leadership and communication skills
  • Ability to work independently and manage multiple priorities
  • Valid driver's license
  • Current CPR certification
