Seeking a Clinical Coordinator to lead our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our South Mountain clinic.
Full-Time Employment: 40 hours per week
Monday- Friday: 8:30am- 5:30pm / Potential for On-Call Weekends as needed

Duties Include:
- Provides clinical and administrative supervision, training and mentoring for Case Managers, Peer Mentors or Peer Support Specialists, and Rehabilitation Specialists on the assigned team.
- Ensures clinical quality and service delivery standards are met or exceeded by direct reports.
- Guarantees that assessments and individual service plans are developed and implemented according to regulatory guidelines for each patient served.
- Provides direct care as needed by assisting patients and assuring the team provides supportive services to help patients reach their goals.
- Collaborates with external service providers, including medical providers, to assure best care and coordination is provided to patients.
- This role reports directly to the site Health Center Director.
