    Description

    RISE DBT IOP is a comprehensive Dialectical Behavior Therapy (DBT) Intensive Outpatient Program (IOP) for suicidal and self-harming adolescents in grades 8–12 within CHC’s Clinical Services Division. Developed collaboratively by Children’s Health Council (CHC) and Stanford Children’s Health, RISE DBT IOP supports adolescents and families on the mid-peninsula and is the only comprehensive DBT IOP in Northern California focused on adolescents with acute suicidal and self-injurious behaviors. We are looking for a Clinical Case Manager to join our team.  This position would be a 0.8 Full Time Equivalent position (about 32 hours a week).

    Job Summary 

    As the RISE IOP/DBT Clinical Case Manager, you serve as a primary point of contact for families seeking DBT services at CHC and help guide them through the RISE Intensive Outpatient Program. Working closely with the RISE Clinical Team, Care Coordinators, and the RISE Clinical Program Manager, you support families from their first inquiry through intake, program participation, and discharge planning. 

    Family and Referral Support 

    You respond to inquiries about CHC’s DBT programming, including the RISE Intensive Outpatient Program, from parents, medical and mental health professionals, school staff, and providers from other levels of care. Using strong clinical judgment, empathy, and a deep understanding of RISE services and clinicians, you help families identify services that are appropriate for their needs. 

    Screening, Intake, and Care Coordination 

    You are fully integrated into the RISE IOP team and work closely with clinical staff to understand program details, coordinate client needs, and determine appropriate levels of care. You manage and participate in the screening and intake process, collaborating with families, the clinical team, and current treatment providers to support a seamless transition into RISE IOP. 

    You also support outpatient DBT programming within Clinical Services by managing client screenings, coordinating intake with DBT providers, and helping determineclinical fit and level of care. As families move through treatment and discharge, you partner with RISE clinicians to connect them with appropriate ongoing care within CHC or the community. 

    Program Operations 

    You balance the needs of families, referral sources, and other consumers with the operational needs of RISE. This includes monitoring and reporting trends to the Clinical Program Manager, maintainingstrong communication with internal and external partners, and ensuring that each family’s experience reflects CHC/RISE’s commitment to excellence, compassion, and capable care. 

    Essential Duties and Responsibilities 

    • Respond to all phone and email inquiries about CHC DBT programming, including RISE IOP, live when possible or by the end of the next business day. 

    • Screen for clinical fit by assessing current self-harm, suicidal ideation and behaviors, mood-related symptoms, and other relevant factors; guide families toward RISE, other CHC services, or appropriate external resources. 

    • Participate weekly in the RISE DBT Consultation Team, DBT Training Seminar, and direct supervision. 

    • Assess teen risk factors through detailed discussions with parents, referring providers, and other levels of care regarding self-harm, suicidal ideation and behaviors, substance use, disordered eating, functioning, and current diagnosis; participate in initial and ongoing training to support screening. 

    • Oversee screening and intake appointments for RISE IOP and outpatient DBT providers, including ensuring required paperwork is sent and completed in advance. Once trained in the DBT commitment process, conduct screenings and intakes as an additional program provider. 

    • Coordinate and schedule care coordination calls with external treatment teams to support continuity of care, including sharing clinically relevant referral information as appropriate. 

    • Document all calls, contacts, and relevant data accurately and thoroughly in the Electronic Health Record (EHR) system. 

    • Recognize when inquiries or clinical concerns should be escalated to the Clinical Program Manager. 

    • Build and maintain knowledge of RISE IOP clinical services, CHC services, and community resources through initial and ongoing training and exposure to clinical programming. 

    • Serve as the primary point of contact for families as they transition into RISE IOP and throughout discharge planning. 

    • Coordinate with colleagues and clinicians to ensure timely, effective communication about client care needs and plans. 

    • Use operational planning and interpersonal skills to manage clinician calendars effectively and support enrollment targets. 

    • Coordinate internal referrals in alignment with agency priorities and community partnerships. 

    • Consistently follow up with current and prospective clients, as appropriate. 

    • Track personal and team outcome measures, manage the measurement-informed care process for families, and recommend or implement improvements in partnership with the team. 

    • Maintain high standards of customer service, clinical responsiveness, and operational efficiency. 

    • Support the RISE IOP team with other administrative or clinical duties as assigned. Given the dynamic nature of IOP programming, opportunities to provide direct services, including groups or individual therapy, may be available. 

    Education and Qualifications:  

    • APCC/AMFT/ACSW from an accredited graduate school, preferably with at least 1 year of experience in either/both a pediatric clinic and/or school-based setting.  

    • Valid California associates license to practice in a mental health field required. 

    • Adherence to all professional, ethical, legal, and confidentiality issues outlined by the California Board of Behavioral Sciences, American Association of Marriage and Family Therapy or National Association of Social Workers, and State and Federal laws (e.g., HIPAA). 

    Knowledge/Ability/Skills 

    • Strong knowledge of medical and behavioral health terminology. 

    • Able to communicate medical and behavioral health concepts verbally and in writing. 

    • Experience or ability/comfort to assess, track and communicate about multi-diagnostic, high-risk clients and behaviors including self-harm, suicidality, substance use and disordered eating.  

    • Knowledge and understanding of CHC's four areas of expertise: ADHD, LD, anxiety and depression and Autism Spectrum disorders 

    • Excellent interpersonal and communication skills, as well as strong customer service orientation.  

    • Excellent time management and organizational skills. 

    • Strong ability to prioritize and rapidly switch tasks within a fast-paced environment. 

    • Attention to detail and accuracy. 

    • Knowledge and use of computers is essential. 

    • Familiarity withElectronic Health Record (EHR)systems; training will be provided.   

    • Growth mindset and openness to continuous improvement. 

    • Ability to work in team-oriented environment. 

    • Knowledge of behavioral health services very desirable. 

    • Bilingual – English, Spanish and/or Mandarin an added plus. 

    Physical and Special Job Requirements:

    • An employee in this position may be required to occasionally lift, move and/or set-up stacks of documents lap-top computers, in-focus projectors, flip charts and other materialsweighing up to 20 pounds.  Employee will also be required to work for extended periods of time at a computer terminal. 

    • To meet the needs of our clients, flexibility in work schedule is required.  

    • RISE IOP runs Monday through Thursday with afternoons and evening availability up to 6:15pm.As case manager, there is some flexibility with work schedule.   

    • Compensation listed is representative of a 1.0 FTE (40 hour work week). The actual compensation for this role will be adjusted to start at 0.80 FTE (20-32 hour/week schedule)

    • Frequent interpersonal interaction with peers, licensed clinicians, clinical program manager, and outside contacts.
