Clinical Case Manager
Behavioral Health
Lincoln Community Health Center, Inc., a federally qualified community health center located in Durham, NC is recruiting for a Clinical Case Manager for Behavioral Health. Durham is known as the Bull City, the City of Medicine, the Tastiest Town in the South, and the Startup Capital of the South. It's a two-hour drive from the beach, a three-hour drive from the Blue Ridge Mountains, and is North Carolina's fourth largest city by population.
Position Description
Provides bio-psycho-social-spiritual assessment with patients referred to Behavioral health; develops team-based treatment plan and interventions designed to improve patient health; works with team to implement treatment plan; research resources, refer, link, coordinate services to ensure client access to all available resources to meet identified needs. Provide education, skill-building, group facilitation and therapeutic interventions as needed.
Knowledge, Skills and Abilities
Ability to communicate clearly in writing and verbally in a professional manner and style, and the ability to communicate effectively with people of diverse backgrounds and educational levels. Ability to maintain an effective working relationship with patients, staff, community partners and the general public. Knowledge of or ability to obtain knowledge of community resources. Strongly prefer ability to communicate in English and Spanish.
Education
Master's degree in Human Service area such as social work or psychology; at least an Associate license in social work, psychology, or counseling.
Previous Experiences
Two years documented experience in the field of mental/behavioral health or case management in behavioral health, medical or social service agency.
