Clinical - Care Manager

Immediate need for a talented Clinical - Care Manager. This is a 03+months contract opportunity with long-term potential and is located in Central Kansas: Salina, Hutchinson, Great Bend, Pratt, and surrounding areas(Onsite). Pay Range: $36/hr - $40/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• 48-hour in-person visits following placement changes, engaging in 7-day team meetings, and performing telephonic outreach based on member acuity.
• Conduct in-person visits and assessments based on member acuity.
• Coordinate services across child welfare and related systems of care.
• Participate in team meetings to discuss member progress and strategies.
• Maintain high-quality documentation and demonstrate strong communication skills.
• Adapt to the needs of members with flexibility and solution-focused approaches.

Key Requirements and Technology Experience: 

• 2 – 4 years of related experience in behavioral health or social work.
• A Master's degree in Behavioral Health or Social Work, or a degree from an Accredited School of Nursing.
• Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements with BH experience required.
• At least 1 year of experience coordinating services across child welfare and related systems of care.
• Strong communication skills, high-quality documentation, adaptability, and a solution-focused mindset.
