Pay Range: $37 - $40/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Review and prioritize new member referrals received daily through TruCare.
• Conduct timely telephonic outreach to eligible members, explaining Care Management services and engaging them in the program.
• Complete comprehensive member assessments and identify barriers to care.
• Develop and maintain individualized care plans based on member needs.
• Coordinate healthcare services and connect members with appropriate resources.
• Provide disease-specific education and health coaching.
• Encourage completion of annual wellness exams, preventive screenings, and immunizations.
• Identify and address gaps in care and opportunities for improved health outcomes.
• Address medical needs, behavioral health concerns, and social determinants of health.
• Escalate urgent or complex concerns appropriately.
• Maintain accurate, timely, and thorough documentation in Care Management systems.
• Collaborate with providers, members, caregivers, and internal teams to coordinate care.
• Work independently while managing a high-volume caseload of 75 members, completing approximately 400 calls per month.

Key Requirements and Technology Experience:

• Key Skills; Case management experience preferred.
• ASN or BSN degree.
• At least 1 year of nursing experience.
• Active Registered Nurse (RN) license.
• Clinical Assessment & Critical Thinking skills to assess member needs and determine appropriate interventions.
• Strong verbal and written communication skills for effective member engagement.
• Time Management & Organization abilities to manage a caseload of 75 members and meet deadlines.
