Job Type: Direct Hire

Salary:  
$80,000.00 - $90,000.00 annually
Up to a 10% annual bonus
 $5,000.00 Sign-on bonus

Job Summary

We are seeking a compassionate and organized Registered Nurse (RN) to support members with care coordination, referrals, education, and follow-up. This role will work closely with patients, providers, and care teams to improve the overall patient experience and health outcomes.

Key Responsibilities

  • Provide in-person and virtual support, education, and care coordination.
  • Coordinate specialist and diagnostic referrals, appointments, and follow-ups.
  • Support members with chronic conditions through education and outreach.
  • Identify and address gaps in care and other clinical needs.
  • Assist with quality programs and patient engagement initiatives.
  • Communicate referral requirements and information to the Care Team.
  • Manage service and scheduling requests on behalf of members.
  • Build strong relationships with healthcare providers and community organizations.
  • Maintain accurate records and patient confidentiality.

Required Skills

  • Active, unrestricted Arizona RN License.
  • Active Arizona Fingerprint Clearance Card.
  • Current CPR Certification.
  • Bachelor's Degree in Nursing (BSN).
  • Case/Care Management experience.
  • Electronic Medical Records (EMR/EHR) experience; no specific system required.
  • Strong customer service and communication skills.
  • Excellent organization and attention to detail.
  • Empathy, patient advocacy, and collaborative approach.
  • Valid Arizona Driver’s License, acceptable driving record, and reliable transportation.
  • Ability to maintain strict patient confidentiality
