A well-established, stable hospice organization is seeking an experienced Clinical Care Director to oversee daily clinical operations committed to delivering exceptional end-of-life care throughout the Houston area.

What You'll Do

As the Clinical Care Director, you will provide leadership and oversight for all clinical operations, ensuring patients and families receive the highest quality hospice services while maintaining compliance with state and federal regulations.

Responsibilities include:

  • Lead and mentor an interdisciplinary clinical team including RNs, LVNs, Hospice Aides, Social Workers, Chaplains, and other clinical staff.
  • Oversee patient admissions, care planning, and ongoing clinical services.
  • Ensure compliance with Medicare Conditions of Participation, state regulations, and accreditation standards.
  • Facilitate interdisciplinary team (IDT) meetings and promote collaborative patient care.
  • Monitor quality outcomes and lead performance improvement initiatives.
  • Support recruiting, hiring, orientation, coaching, and performance management of clinical staff.
  • Review clinical documentation for accuracy and regulatory compliance.
  • Assist with staffing, scheduling, productivity, and clinical resource management.
  • Partner with executive leadership to achieve operational and clinical goals.
  • Participate in the clinical on-call rotation as needed.
  • Foster a culture centered on compassion, accountability, and excellence.

What We're Looking For

  • Current Texas Registered Nurse (RN) license.
  • Minimum of three years of hospice leadership experience.
  • Previous experience supervising clinical teams.
  • Strong knowledge of hospice regulations, Medicare Conditions of Participation, and survey readiness.
  • Excellent leadership, communication, and organizational skills.
  • Ability to effectively coach, mentor, and develop clinical staff.
  • Experience with EMR documentation systems preferred.
  • Valid driver's license and reliable transportation.
