Clinic Oncology Nurse Navigator - Topsham

Participate in interdisciplinary team conferences, in-services and continuing education programs, national and regional professional organizations and community sponsored programs specific to Oncology and Hematology. The Clinic Oncology Nurse Navigator will interact with patients, offer advice and counsel to patients, as well as advise physicians of patient’s needs and recommended interventions.

NECS is currently seeking a part-time Clinic Oncology Nurse Navigator in our Topsham location to help the patients and families navigate along the health care continuum. The Clinic Oncology Nurse Navigator will interact with patients, offer advice and counsel to patients, as well as advise physicians of patient’s needs and recommended interventions. Perform nursing assessments and the triage of patient care needs over the phone and in the office. This position works collaboratively with physicians, APP's, RN peers, Patient Care Coordinators, and other clinical and non-clinical personal within and outside the practice

Essential Duties and Responsibilities:

• Point of contact for referring physicians, patients, and caregivers to assist with patient scheduling and coordination of care.
• Assist physicians; conduct patient assessments.
• Receive patient telephone calls and intervene appropriately for that patient’s specific needs in a timely manner. Alert physician as needed to patient needs that require physician intervention.
• Follow up with patients as needed by telephone, to ensure compliance with treatment guidelines and to evaluate changing or uncertain clinical situations.
• Draw blood from ports, central lines, and peripheral IV access. Perform other procedures as needed.
• Ensures follow-up appointments and tests/procedures are scheduled for patients.
• Maintain patient care standards and assures that patient care functions, such as assessment, treatment and documentation are completed safely and effectively.
• Maintain open and effective communication with all physicians, employees and vendors.
• Participate in interdisciplinary team conferences, in-services and continuing education programs, national and regional professional organizations and community sponsored programs specific to Oncology and Hematology.
• Incorporates evidence-based research into clinical practice and provide recommendations on practice clinical policies and processes to practice Directors.
• Coordinate patient care responsibilities within the scope of the practice for licensed and unlicensed health care providers.
• Demonstrates respect for life, dignity, and rights of all persons.

Travel: Very Minimal. Might need to travel to other locations occasionally, on an as needed basis for coverage.

Weekends: Occasional, these positions will participate with other nurses in the weekend urgent care coverage rotation.

Yearly Salary $68,640-$114,400

Required Experience:

• Bachelor degree in Nursing preferred. Must be enrolled in a BSN program within 6 months of hire, and complete you degree within 4 years of hire.
• Licensed Nurse within State of Maine required
• OCN Certification Preferred
• Two years of Hematology/Chemotherapy/Oncology nursing experience preferred
• ONS Chemotherapy & Bio-therapy Certification required, or have the ability to get certification within one year of employment.
• Two - Five years plus nursing professional experience required.
• Must have strong IV skills.
• Ability to communicate effectively with patients, physicians and staff
• Must be detail oriented, strong follow-up skills, and able to manage multiple priorities.
