Advocacy for patients and their significant others remains a core component of the role, ensuring their needs are addressed effectively and sensitively.
Key Responsibilities:
Provides quality primary healthcare and wellness mobile health services to targeted populations.
Collaborates with primary care providers, specialty providers, and other agencies to develop comprehensive care plans tailored to individual needs.
Identifies expected outcomes and develops plans to address patient problems, collaborating with treatment team members on the patients care plan. Plans are individualized in partnership with the patient, family, and others, respecting their values, beliefs, and preferences. Consults with physician specialists or designees as necessary and coordinates care and services with the primary care provider.
Provides anticipatory guidance to individuals, families, groups, and communities aimed at promoting health and preventing or reducing the risk of health problems. Coordinates and facilitates community education and screening initiatives to enhance health outcomes.
Identifies patient teaching needs and either directs to appropriate programs or relates information to the patient one-on-one. Provide patient education of disease process & management.
Continually develops professional skills as a Certified Nurse Practitioner by attending education programs or CME conferences. Actively participates in professional organizations. Support and educate treatment team members. Contributes to nursing knowledge by conducting or synthesizing research and other evidence that discovers, examines, and evaluates current practice, knowledge, or theories to improve healthcare outcomes. Participates in and supports the precepting and mentoring processes.
