    About the Role

Our Rapid Rehousing program combines rental assistance, housing navigation, and supportive services to help people secure and retain permanent housing. We follow Housing First: housing access is not conditioned on employment, income, sobriety, or criminal history. We need someone who can manage a typical caseload of 20 to 25 clients using trauma-informed, strength–based, culturally responsive care and harm reduction. This role is a strong fit for someone who enjoys community-based work, builds trusting relationships, and independently organizes a changing weekly schedule while following through on client goals and documentation.

    What You’ll Do

    • Assess strengths, housing barriers, service needs, and program eligibility; partner with clients to develop and update individualized Housing Stability Plans.

    • Maintain weekly client communication and coordinate services that support housing retention, income growth, and client-defined goals.

    • Support housing searches, lease-up, and landlord communication; coordinate rental assistance and address barriers to prevent eviction and sustain housing.

    • Use motivational interviewing, active listening, and culturally responsive advocacy to engage clients; provide crisis intervention and de-escalation when urgent needs arise.

    • Connect clients with employment, benefits, healthcare, and community resources in collaboration with internal teams and service partners.

    • Conduct home visits and community meetings; transport clients, assist with vehicle access and mobility as needed, and deliver welcome packages into homes when necessary.

    • Maintain accurate case notes and client records in the Homeless Management Information System (HMIS) and other required databases, following U.S. Department of Housing and Urban Development (HUD) standards. Complete session notes by week’s end or the following Monday.

    How You’ll Work

    • Organize your weekly client visits, outreach, and documentation independently; adapt to changing needs while meeting service expectations.

    • Receive weekly supervision and participate in biweekly team meetings and monthly departmental meetings.

    • Work includes computer-based documentation, client transportation and mobility assistance, and lifting or carrying welcome packages and supplies up to 50 lbs., with reasonable accommodations considered.

    What You Bring

    Required:

    • Bachelor's degree in human services, Social Work, Psychology, Education, Business, or related field.

    • At least two years of experience in case management, homeless services, or supporting vulnerable populations, including experience with trauma-informed care and Housing First.

    • Strong assessment, communication, collaboration, motivational interviewing, and de-escalation skills; a client-centered approach grounded in harm reduction.

    • Ability to prioritize competing needs, manage your schedule independently, and maintain timely, accurate records; proficiency in Microsoft Office and documentation systems.

    • Valid driver’s license and ability to travel in the community and transport clients as needed.

    Preferred:

    • HMIS experience, knowledge of Chicago housing resources, and experience with landlord engagement or housing placement.

    Compensation & Benefits

The pay scale for this role ranges from $22.20 - $26.64 per hour.

    Benefits include:

    • Medical, dental, vision, and life insurance; flexible spending and health savings accounts.

    • 403(b) retirement plan with employer match and generous paid time off.
