We are looking for a highly organized, compassionate, and operationally minded Licensed Practical Nurse (LPN) to join our leadership team as a Client Care Manager.

     

    Position Overview

    As the Client Care Manager, you will be the operational backbone of our client services and field caregiver team. In this dynamic role, you will lead client intakes, coordinate day-to-day operations, manage caregiver schedules, and act as the primary liaison between families and our care team.

     

    Key Responsibilities

    • Client Intake & Care Coordination: Act as the first point of contact for families seeking care. Conduct non-clinical service consultations, introduce caregivers, and seamlessly coordinate the startup of care.
    • Daily Operations & Triage: Act as the primary point of contact for field caregivers reporting client changes in condition, utilizing established clinical triage protocols.
    • Caregiver Management & Scheduling: Oversee, mentor, and evaluate field staff (CNAs, HHAs, and Companions). Coordinate with the scheduling team to ensure seamless shift coverage and zero service gaps.
    • RN & Family Liaison: Serve as the critical link between the field and our RN Supervisor. Escalate client changes in condition to the RN for formal Care Plan adjustments and keep family members proactively updated.
    • Compliance Maintenance: Ensure client files, caregiver records, and daily logs remain up to date and audit-ready in alignment with Maryland Office of Health Care Quality (OHCQ) guidelines.

     

    Qualifications & Skills

    • Active Maryland LPN License: Must possess a current, unencumbered license as a Licensed Practical Nurse in the State of Maryland (required).
    • Home Care Experience: Minimum of 1–2 years of experience working in a home care, home health, assisted living, or long-term care setting (preferred).
    • Strong Leadership Capabilities: Proven experience managing, scheduling, or coordinating teams of field staff or caregivers.
    • Excellent Communication: Exceptional verbal and written communication skills with the ability to navigate difficult conversations with stressed families with empathy and professionalism.
    • Tech-Savvy: Proficiency with home care management software (e.g., ClearCare, HHAeXchange, AxisCare) and Microsoft Office Suite.
    • Reliable Transportation: Valid Maryland driver’s license, clean driving record, and a reliable, insured vehicle for local community client visits.
