What You'll Do

  • Develop and implement transition plans aligned with IEP goals
  • Conduct transition assessments to identify strengths and interests
  • Support students in career readiness and independent living skills
  • Build relationships with community agencies and local employers
  • Educate families and staff on transition resources and pathway

What You Bring

  • Bachelor's degree in Special Education, Counseling, or Human Services
  • CT Certification (165, 068, or 071)
  • Experience supporting students with disabilities
  • Knowledge of community resources and vocational services
