    Position overview

    Above All Talent Solutions is partnering with a rapidly growing therapy provider that delivers innovative rehab programs to skilled nursing and long-term care communities across the U.S. This full-time COTA position may have potential to support nearby sister facilities, serving adult and geriatric residents in a post-acute/long-term care setting.

    As a key member of an interdisciplinary rehab team, you will assist OTs in helping residents improve daily living skills, mobility, and independence to maximize safety, function, and quality of life.


    Responsibilities

    • Implement individualized OT treatment plans for adult and geriatric residents with physical, cognitive, and functional limitations in a SNF/LTC environment under OT supervision.
    • Provide hands-on therapeutic interventions, including ADL training, therapeutic activities, and adaptive equipment use in collaboration with nursing, PT, SLP, and medical staff.
    • Complete timely, accurate documentation of treatments, progress notes, and resident response per facility, payer, and regulatory standards.
    • Educate residents, families, and staff on adaptive techniques, safety measures, and home modification strategies.
    • Participate in care conferences, QI initiatives, and program development to support strong clinical and operational outcomes.

     

    Qualifications

    • Current, unrestricted COTA license in the state of practice (or eligibility prior to start).
    • NBCOT certification preferred; strong new grads with relevant LTC/acute rehab experience may be considered based on client need.
    • Prior experience in skilled nursing, post-acute rehab, or long-term care strongly preferred.
    • Excellent communication, documentation, and collaboration skills; resident-centered, outcomes-focused approach.

     

    Compensation and benefits

    The hiring facility offers a competitive hourly rate (market range in nearby northern Minnesota markets is typically mid-30s to low 40s per hour, depending on experience) plus a comprehensive benefits package. Full-time benefits include:
