Cupp 4 South is a dynamic, fast-paced 32-bed inpatient medical/surgical unit primarily serving the adult population of West Philadelphia. This floor offers the practitioner the opportunity to impart lasting change on the lives of its patients through its team-based nursing approach, as well as through the close relationships forged between other services, including Physicians, Pharmacists, Physical Therapy, Respiratory, Speech-Language Pathology, and Case Management, all of whom play a crucial role in improving patient health and navigating the care continuum. Some of the common patient diseases seen on the floor include COPD, CHF, pneumonia, GI-related illnesses such as Crohn’s disease, Diabetes, renal failure, sepsis, end-of-life, sickle cell disease, and drug and alcohol withdrawal. The 4 South staff also provide specialty care in Oncology and Peritoneal Dialysis. Job Summary:
The Certified Nursing Assistant (CNA) plays a vital role in delivering quality care under the direct supervision of a registered nurse (RN). This position involves assisting with patient assessments, providing hands-on care, and collaborating with healthcare teams to ensure the best possible care for patients.
Key Responsibilities:
Patient Report & Communication
- Receive and give verbal/written reports with the RN at the start and end of each shift.
- Report any changes in patient condition to the RN and the next shift team.
Patient Assessment Assistance
- Help collect data such as vital signs, blood sugar, weight, and functional status.
- Observe and report skin changes, bruising, or any other abnormalities.
- Assist in planning care and setting realistic patient goals with the RN.
Care Implementation & Monitoring
- Follow infection control practices and standard precautions.
- Monitor patients at risk for falls, confusion, or other safety issues.
- Assist with daily activities like bathing, grooming, feeding, and transferring.
- Observe and record vital signs, intake and output (I&O), pain levels, and other patient data.
- Report patient findings and concerns to the RN in a timely manner.
- Assist with treatments, including colostomy irrigations, enemas, and oxygen therapy (under RN supervision).
Collaboration & Teamwork
- Participate in patient care rounds and collaborate with healthcare team members.
- Report any issues or changes to the RN or Charge Nurse.
- Attend staff meetings and work cooperatively with the team.
Safety & Environment
- Ensure a clean, safe environment for patients.
- Follow safety protocols, including the use of restraints when needed.
- Promote patient comfort and assist with mobility and repositioning to prevent skin breakdown.
Specialized Tasks
- Assist with patient education as directed by the RN.
- Support post-mortem care and assist physicians during physical exams.
Required Qualifications:
Credentials:
- Basic Cardiac Life Support (BCLS)
- CPR Certification (American Heart Association)
- PA State-certified Nursing Assistant or enrolled in an accredited ASN/BSN program with a GPA of 3.0 or higher.
Education:
- High School Diploma or GED required.
- Graduate of a certified Nursing Assistant program or matriculated in an accredited ASN/BSN Nursing Program preferred; minimally completed the nursing foundations/fundamentals clinical course with a 3.0 GPA overall and in the nursing major.
