Schedule
Working full time, 40 hours per week, 12-hour shifts from 6:30pm to 6:30am (Night Shift).
Assist individuals with transfers, ambulation, eating, bathing, dressing, toileting, personal hygiene, and other activities of daily living (ADLs).
Use simple procedures and devices to strengthen limbs and maintain flexibility as instructed.
Support nursing staff with simple medical procedures such as administering suppositories, enemas, treatments, post-surgical care, and post-mortem care.
Monitor physical and mental status by taking vital signs, obtaining samples, and tracking input/output using appropriate medical equipment and techniques.
Responsible for medical record and worksheet documentation for all assigned individuals, maintaining records, and preparing simple reports.

Maintain cleanliness by changing bedding, linens, and other items to ensure a neat and sanitary environment.
Assist licensed nurses in emergency situations rapidly and appropriately, following established policies and procedures.
Perform other related duties such as transporting individuals to appointments outside the facility and clerical tasks like answering phones and filing.
Actively participate in processes such as Huddle Boards, Problem Solving, Job Breakdown Sheets, Success Tracking, and Standardized Work.
Fully participate in the Quality Assurance and Performance Improvement (QAPI) program mandated by CMS.
