Job Responsibilities • Provide various care services necessary for attending to the personal needs • Support the health of Community Care program members • Serve as coordinator of care for scheduled weekend and holiday shifts • Ensure care and service delivery are in conformance with regulations, policies, procedures, and standards established by Community Care
Certified Nursing Assistant CNA
Job Requirements • Education: High School diploma or GED preferred • Wisconsin Certified Nursing Assistant license in good standing on registry • Experience: Six months of experience providing personal care services for elderly or disabled adults in a group home, assisted living, or long-term care environment and experience working with individuals with dementia
Certified Nursing Assistant CNA Job Schedule • Hours of Operation: Monday - Sunday, Night Shift 10:45 p.m. to 6:45 a.m., 8 hours per week • Every other weekend • Every other holiday
