Position Overview
The Home Health Aide (HHA) / Certified Nursing Assistant (CNA) is a paraprofessional member of the home care team who provides personal care services to patients under the supervision of a Registered Nurse or therapist.
In this role, you will support patients with daily activities, monitor their condition, and communicate observations to the clinical team to ensure safe, high-quality care.
Responsibilities
- Provide personal care assistance such as bathing, grooming, dressing, and mobility support
- Observe and report changes in patients’ condition to the supervising nurse or therapist
- Document care provided and patient observations accurately
- Follow the care plan established by the Registered Nurse
- Maintain a safe and supportive environment for patients in their homes
- Promote quality, continuity, and safety of patient care
Qualifications
- Certified Nursing Assistant (CNA) in Massachusetts preferred; Home Health Aide certification accepted
- Must be 18 years of age or older
- Ability to read and follow written care plans and document services provided
- Ability to work independently with minimal supervision
- Compassion and empathy for individuals who are ill, injured, or elderly
- Current CPR certification required
- Strong interpersonal skills, patience, and professionalism
- Valid driver’s license and reliable insured vehicle
- Ability to assist patients with repositioning, transferring, and other physical tasks.
