Schedule: Wednesday 2:30pm-10:30pm, Thursday 2:30pm-10:30pm, Friday 2:30pm-10:30pm, Saturday 8:30am-10:30pm
The Certified Nursing Assistance is responsible to provide assistance to the support staff with the daily activities of individuals, provide ongoing support, guidance, role modeling and providing assistance in meeting clients medical and health needs.

Responsibilities
- Report to shift on time ready to spend at least 95 percent of working hours in personal contact with individuals
- Serve residents in a manner conducive to their safety and comfort, under the direction of a Program Manager and consultative support of DD Community RN
- Perform all duties in accordance with established methods, techniques and recognized standard
- Assist program support staff in completion of all daily/shift tasks, including but not limited to oversight of individuals’ medical and health needs: hospitalization, appointments, assessments, discharging, transferring and caring for the residents' personal belongings
- Prepare summaries of interaction and objective behavioral observations of clients at the end of shift; clinical notes and data collection using Microsoft Outlook.
- Lift and transfer individuals as needed.
- Assist individuals in Self-Advocacy, decision-making and empowerment.
- Provide ongoing support, guidance and role modeling for individuals.
- Assist individuals with home maintenance and perform cleaning duties as assigned
- Perform Activities of Daily Living (ADLSs) for residents as required
- Coordinate scheduling for residents requiring OT, PT and ST
- Take and record weights, blood pressures, temperatures, pulse and respiration and give skin care treatments, as permitted by state regulation
- Report on resident status or family concerns to Program Manager
- Ensure that residents and families receive the highest quality of service in a caring and compassionate atmosphere, which recognizes the individual's needs and rights
- Perform other duties as requested
- Maintain certification in CPR, First Aid, SOLVE, MAP, HR/DPPC
- Ensure that individuals we support are treated with dignity and respect in accordance with Advocates’ Human Rights Policy.
Qualifications
- Certified Nursing Assistance Certificate Required. Experience with the program population is preferred.
- Ability to write routine reports and correspondence.
- Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
- Ability to read and interpret documents such as policy and procedure manuals.
- Basic computer knowledge.
- Excellent customer services and communication skills.
- Ability to read and speak English, bilingual preferred.

- Must have valid driver’s license and access to auto.
