Under the direction of the Adult Day Program Supervisor, the Certified Nursing Assistant (CNA)/Adult Day Program Instructor provides instruction, personal care, and direct support to adults with intellectual and developmental disabilities. The position promotes participant safety, dignity, choice, community inclusion, and increased independence through meaningful activities aligned with each participant's Individual Program Plan (IPP) and Individual Service Plan (ISP).
Las Trampas operates an Adult Day Program with behavioral and medical day services components. This is a community-based developmental program rather than a long-term care setting. CNAs apply their caregiving skills while also teaching classes, supporting community outings, and encouraging participants to take an active role in daily life. Staff receive training in Medical Day Services and rotate throughout the program based on participant needs and assignments.
Qualifications:
- High school diploma or equivalent.
- Current California CNA certification.
- At least 18 years of age.
- Valid driver's license, reliable transportation, and a driving record meeting agency requirements.
- Ability to obtain required criminal background clearance.
- One year of experience supporting individuals with developmental disabilities preferred.
- Understanding of person-centered services and positive behavior support principles.
- Effective written, verbal, interpersonal, and computer skills.
- Sound judgment, problem-solving ability, personal accountability, and professional conduct.
- Ability to obtain and maintain required CPR, First Aid, and agency training certifications.
Responsibilities and Duties:
Instruction and Person-Centered Support
- Develop activity schedules and lesson plans based on participant interests, preferences, abilities, and established goals.
- Provide instruction in daily living skills, art, cooking, health and wellness, music, communication, social skills, exercise, and vocational skill development.
- Use approved curricula, adapt teaching methods to individual needs, prepare materials, and maintain an organized learning environment.
- Ensure activities align with IPPs, ISPs, the ADP program design, applicable requirements, and participant choice.
- Encourage participation while respecting each individual's communication, decisions, and right to decline.
- Provide instructional assistants with guidance and feedback on implementing activities and curricula.
- Document activity changes and obtain supervisory approval for changes requiring authorization.
Personal Care, Health, and Safety
- Assist participants with toileting, brief changes, dressing, hygiene, oral care, feeding, positioning, transfers, and mobility according to individual support plans.
- Provide personal care throughout the program day, including lunch and transitions between activities.
- Apply proper body mechanics, infection prevention practices, standard precautions, and safe use of adaptive equipment.
- Follow individualized feeding instructions, dietary restrictions, allergy precautions, and other approved care procedures.
- Perform assigned health-related support only within applicable role limits, documented training, approved care plans, and required nursing direction.
- Observe and promptly report changes in health, skin condition, behavior, mobility, or support needs. Complete assigned skin observations, seizure logs, and other care documentation.
- Maintain supervision of assigned participants until another authorized staff member assumes responsibility.
- Respond to emergencies according to agency procedures and training.
- Recognize and report suspected abuse, neglect, and other concerns according to agency reporting procedures.
Community Participation and Behavioral Support
- Support community learning and outings using Las Trampas vehicles and public transportation, including BART and buses.
- Follow vehicle safety procedures and agency requirements for handling participant and company funds.
- Implement approved positive behavior support plans consistently.
- Communicate concerns and recommendations to the supervisor; obtain authorization before changing established behavior interventions.
- Promote independence by supporting participants to complete tasks and make choices to the greatest extent possible.
Documentation and Professional Responsibilities
- Complete accurate, timely daily notes, progress data, assessments, behavioral records, and other required documentation.
- Communicate participant needs and concerns to supervisors and appropriate team members.
- Maintain confidentiality, professional boundaries, and respectful relationships with participants, families, coworkers, and community members.
- Attend required meetings and training, maintain required certifications, and participate in monthly CNA continuing education opportunities provided through Las Trampas.
- Follow agency policies, applicable requirements, and the Las Trampas mission.
- Complete other duties as assigned.
Work Environment and Opportunities:
This position follows the DSP pay scale, with core responsibilities and performance expectations consistent with the DSP role. Staff work collaboratively, with classrooms averaging approximately three to four staff, depending on program assignments and participant needs.
The position offers opportunities to apply CNA skills, teach a variety of subjects, participate in community activities, and pursue advancement to lead or coordinator roles as opportunities become available. Monthly CNA continuing education opportunities are provided. Staff wear attire consistent with agency dress requirements; scrubs are not worn, supporting participants' comfortable inclusion in everyday community settings.
