Certified Nursing Assistant (CNA) – Congregate Living Health Facility (CLHF)


Position Summary


We are seeking a compassionate, dependable, and motivated Certified Nursing Assistant (CNA) to join our team in a Congregate Living Health Facility (CLHF) serving residents with complex medical needs.


Under the supervision and direction of a Licensed Vocational Nurse (LVN) or Registered Nurse (RN), the CNA provides direct resident care, assists with activities of daily living (ADLs), observes and reports changes in resident condition, and helps maintain a safe, clean, respectful, and comfortable care environment.


Essential Duties & Responsibilities


  • Assist residents with activities of daily living, including bathing, grooming, dressing, toileting, and personal hygiene.
  • Provide feeding, hydration, and meal assistance based on each resident’s individual needs and care plan.
  • Measure, record, and report vital signs, including temperature, pulse, respirations, and blood pressure.
  • Assist residents with transfers, repositioning, mobility, and ambulation using proper body mechanics and appropriate equipment.
  • Observe residents for changes in physical, emotional, behavioral, or mental condition and promptly report concerns to the LVN or RN.
  • Complete accurate and timely documentation of care and services provided.
  • Keep resident rooms, care areas, and equipment clean, organized, and safe.
  • Follow facility infection prevention and control practices, safety policies, and emergency procedures.
  • Respond promptly and professionally to resident call lights, requests, and care needs.
  • Protect and promote resident dignity, privacy, confidentiality, and rights at all times.
  • Work collaboratively with nurses, CNAs, and other members of the healthcare team to provide consistent, high-quality resident care.
CLHF Care Environment


Our facility provides care for medically complex residents who may have conditions or care needs including:


  • Ventilator dependence and respiratory support
  • Neurological disorders
  • Spinal cord injuries
  • Mobility limitations
  • Chronic and complex medical conditions
  • Residents requiring extensive assistance with activities of daily living
CNAs must be comfortable working around medical equipment and providing hands-on care to residents with varying levels of physical dependency.


Qualifications


  • Current and active California CNA certification in good standing
  • Current CPR/BLS certification preferred
  • Knowledge of basic nursing care, resident safety, and infection prevention practices
  • Ability to communicate effectively in English; bilingual skills are a plus
  • Ability to safely assist with lifting, transferring, repositioning, and mobility
  • Strong interpersonal, communication, and teamwork skills
  • Compassionate, dependable, and respectful approach to resident care
  • Ability to follow instructions and promptly communicate changes or concerns to licensed nursing staff
Physical Requirements


Candidates must be able to:


  • Stand and walk for extended periods throughout the shift
  • Frequently bend, reach, lift, reposition, and assist residents
  • Lift up to 50 pounds and assist with heavier resident transfers using appropriate equipment and assistance
  • Safely perform physically demanding resident-care responsibilities
  • Respond promptly during emergencies and urgent resident-care situations
  • Work effectively in a fast-paced healthcare environment
Work Environment


This position is based in a Congregate Living Health Facility (CLHF) providing care to residents with complex medical needs.


The position may involve exposure to medical equipment, bodily fluids, and potentially infectious materials. All employees are expected to follow applicable OSHA requirements, infection control practices, and facility safety policies and procedures.
