Trinity Health Of New England At Home is a national home care, palliative care, and hospice organization. We provide comprehensive home care and end-of-life services in the comfort of patients homes. We care for patients and their families through skilled nursing, therapy services, medical social work, hospice care, and bereavement support.
Position Purpose: Our home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the states Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures. Must be a Certified Nurse Assistant in the state of Connecticut.
Position Details: Schedule also includes two rotating holidays per year. Sign on Bonus: $5,000 paid out over two years. This position is 100% home care assignments working with hospice patients.
