    Location: Fair Oaks Health Care Center– Crystal Lake, IL (on-site position) 
    Status: Full-Time | Part-Time | PRN  
    Schedule: Days | PMs | NOCs | Weekends – True Flexible scheduling - you choose what fits your schedule.  Currently we have openings for Part-Time 7a-3p, Full-Time 3p-11p.  Weekend Warriors (which comes with a $5 Weekend Warrior Differential - 3p-11p and 11p-7a.)  Weekend Warriors have the option of working every other weekend or every weekend.

    Job Summary: 
    The Certified Nursing Assistant (CNA) will provide patient/resident care as required under the direct supervision of the licensed nurse. The CNA at Fair Oaks Health Care Center completes assigned tasks to ensure timely, efficient and safe patient/resident care. Communication and interactions with patients, residents, families and other health team members is an essential component of this position.

    Compensation: 
    • CNA base hourly wage: $20.00 – $22.00 per hour dependent on experience 
    • PLUS shift differentials - $2.00 per hour PM shift and $2.50 per hour NOC shift

    Key Responsibilities: 
    • Performs initial and ongoing data collection on all patients/residents under the direction of the Registered Nurse/Licensed Practical Nurse. 
    • Implements plan of care developed by the Registered Nurse based on patient/resident physical, psychosocial, cultural, ethnic and religious/spiritual needs. 
    • Provides direct patient/resident care and reports condition and changes to RN or LPN. 
    • Demonstrates time management and prioritization of tasks. 
    • Document results of patient/resident care within established policies, procedures and scope of practice. 
    • Nutritional care 
    • Effective communication with residents and family 
    • Completes all other duties as assigned.

    Qualifications: 
    Required: 
    • Must be in good standing on the State Nursing Assistant registry or appropriate entity 
    • Ability to meet physical and job-related requirements 
    • Must be English literate and possess excellent written and verbal communication skills 
    Preferred: 
    • Previous hospital or long-term care or skilled nursing experience 
    • CPR Certified
