    Description

    Assisted is looking for a per diem Certified Home Health Aide for our Culver City Home Health branch.

    We have about 10 visits per week. 

    You must have a Home Health Aide Certification through the California Department of Public Health.

    The impact you will make:
    • Assist patient with personal hygiene, elimination, / nutritional support, ambulation and exercise according to the patient’s needs and as specified on the CHHA Assignment.
    • Ensure quality and safe delivery of CHHA services per written plan of care.
    • Communicate with RN, Rehab Case Manager or Clinical Supervisor on a regular basis regarding changes in the patient’s condition and/or needs.

    Who you are: 
    • Completion of an approved training program pursuant to Federal/State law.
    • Current, active California CHHA certificate with criminal background check.
    • Current CPR, health clearance and completed skills assessment.
