Job Summary:

The Wayne Mobile Health Units represent the future of healthcare, providing ‘care your way, where you live and work,’ with the long-term goal of improving population health outcomes. The Wayne Health Mobile Units deliver an ever-expanding menu of health care and social services directly to neighborhoods or places of employment.

The Mobile Health Unit Registered Nurse will provide professional nursing care to patients following established Departmental policies and procedures.

Job Description:

Principle Duties and Responsibilities:

  • Conduct initial patient evaluations including taking vital signs, height/weight measurements, documenting allergies, medications, past medical history, family and social history.
  • Document if patient has Advanced Directives or if educational material is requested.
  • Assist physicians during examinations, perform wound care, administer injections, and assist with procedures such as phlebotomy.
  • Administer oral, intramuscular, and subcutaneous medications as prescribed; maintain medication records; and coordinate pharmacy orders.
  • Administer vaccines (including pediatric) and manage immunization inventories while at MHU sites.
  • Perform or assist with routine lab tests, screenings, and diagnostic procedures.
  • Provide information on treatment plans, medications, self-care, and disease prevention.
  • Alerts provider of any abnormality in vital signs or appearance for further reassessment.
  • Assist in managing medical emergencies, including maternal complications, respiratory care, and first responder training.
  • Coordinate patient MHU visits with Clinical Lead.
  • Maintain accurate, confidential patient records; update electronic health records (EHR) and comply with HIPAA, OSHA, and infection control policies.
  • Prepare and maintain clinical supplies, equipment, and inventory.
  • Liaise with physicians, nurses, visiting nurses, pharmacists, medical students, research assistants and volunteers to ensure continuity of care.
  • Adhere to infection control and safety protocols; maintain compliance with state and federal healthcare regulations.
  • Support workplace safety by following policies for bloodborne pathogens and emergency preparedness.
  • Performs venipuncture and IV therapy according to the physician orders or patient emergencies.
  • Contact the appropriate department or physician’s office when information is missing.
  • Ensure an efficient flow of patients accommodating the needs of the patients and/or providers as they arise.
  • Schedule special laboratory testing, x-rays, consultations with other subspecialty areas as directed, etc.
  • Recognize abnormal laboratory values and test results and notifies provider following written policies.
  • Reinforces the provider's discharge instructions and follow up appointments as well as, calling in prescription renewals to pharmacies for patients as directed by physicians.
  • Assure all medications are charted in EMR.
  • Check encounter forms for level of service, diagnosis, faculty signature, and referring physician if required.
  • Checks diagnostic laboratory forms and provider orders on patient’s record for accuracy and completeness.
  • Instructs or assist patients in specimen collection, tests and procedures.
  • Maintain clinical area stock to ensure the necessary supplies and pharmaceuticals are available as well as ensuring the necessary equipment and instruments are available and notify all problems to the Clinical Operations Manager.
  • Recognize and respond appropriately in life threatening situations.
  • Provide education to patients and families.
  • In conjunction with the provider, contact patients regarding test results and recommended follow up.
  • Maintain a professional appearance.
  • Maintain and protects patient confidentiality according to Departmental and HIPAA guidelines.
  • Responsible and accountable for own practice within the Nursing Practice Act and accepted standards of nursing practice.
  • Responsible for registering patients, collecting insurance, placing orders for providers through order delegation.
  • Perform related duties as assigned.

Education, Licensure/Certification, and Training Required for the Job:

  • Graduation from accredited school of nursing.
  • Current Michigan RN licensure to practice as registered nurse.
  • Knowledge of Public Health Code and Michigan Nurse Practice Act preferred.
  • 1-2 years experience in an Ambulatory Care Setting preferred.
  • Valid Driver’s License.

Skills and Abilities Required:

  • Knowledge of nursing theory and practice to give and evaluate patient care.
  • Maintains professional effective working relationships with patients, medical staff and the public.
  • Reacts calmly and effectively in emergency situations.
  • Ability to work independently.
  • Ability to drive a CARGO VAN.
  • Ability to adapt to working in outdoor elements, rain, snow, heat, in compliance with the departments Weather Policy.
  • Ability to walk or stand for long periods of time.
  • Ability to frequently lift and move Mobile Health Unit equipment weighing up to 40 lbs.
  • Compliance with Department Health and Safety policies and procedures.

Working Condititions/Schedules:

  • Casual position, hours and days (weekends included) to be determined by the business needs of the department.
  • Must be available to work two (2) 6 - 8 hour shifts per week.
  • Must be able to work in a mobile unit environment and office setting.
  • Primarily on-site with office-based with hours when needed.
  • Routinely use standard office equipment such as computers, phones, photocopiers and fax machines.
  • The noise level in the work environment is occasionally noisy when working outside.
  • Must be able to work in outdoor weather conditions when mobile unit events are scheduled outside.
