Casual Physical Therapist - Outpatient

Responsibilities/Job Description:

We are seeking a casual physical therapist to support our outpatient teams at Fairview Lakes Hospital in Wyoming, Fairview Clinic in Chisago and Fairview Clinic in North Branch!

 

The casual PT will treat outpatient general practice PT, ortho and neuro patients. Additional specialties like lymphedema, vestibular, cancer rehab, pelvic health, etc. are welcome. There may be opportunities to support inpatient as well. In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes.

Responsibilities

• The physical therapist partners with the patient and family while performing the Evaluation/Re-evaluation
• Gathers pertinent data
• Performs examination
• Determines treatment diagnosis
• Develops an individualized plan of care through collaboration with the patient and family
• Analyzes data to identify needs and potential risks
• Determines prognosis for recovery
• Establishes individualized goals with patient and/or caregiver
• Individualizes plan of care considering developmental level and cultural differences
• Implements and modifies plan of care based on reassessment and patient response
• Coordinates care and communicates effectively with interdisciplinary team for all aspects of patient care
• Establishes appropriate discharge plan
• Partners with patients and families to provide skilled intervention
• Implements plan of care
• Alters treatment/plan of care to reflect change in patient status and response to treatment
• Assesses patient and/or caregiver learning needs and provides appropriate education

Required Qualifications

• Physical Therapy License in the state of Minnesota or temporary license in the state of Minnesota
• Basic Life Support (American Heart Association or Red Cross)

Preferred Qualifications

• Doctorate of Physical Therapy
• 1 year experience

Qualifications:

$41.59- $60.07 Hourly
