Description

Job Summary/Objective:

Homebased services are a community-based therapeutic service who work with youth and families with several emotional disturbances.  Services are provided within the community.  Homebased caseworkers are members of a small team comprised of a skills coach and therapist who work together to impact goals listed on the youth’s treatment plan.

Essential Duties and Functions:

  • Implement a customized set of strategies, supports and services to meet the individualized goals of the youth and family. 
  • Completion of psychosocial assessments. 
  • Coordination and monitoring. 
  • Education and training.
  • Engages in risk management and completes safety planning.
  • Performs outreach activities within the community.
  • Maintains record compliance according to agency guidelines and standards as well as funding source regulations.   
  • Shall ensure compliance and participation with competency improvement activities including, but not limited to credentialing, continuing education (per licensure requirements or a minimum of 24 hours per year of clinically based trainings), implementation of evidence-based practices, peer review attendance, and professional improvement practices. 

Qualifications:

  • Bachelor’s degree in a human services field is required. Limited or fully licensed Bachelor’s in Social Work, preferred. 
  • Child Mental Health Professional, as defined by the Michigan Department of Health and Human Services provider qualification, is preferred. 
  • One year of computer experience – Word, Excel, and other electronic medical record software preferred.
