    Job Summary

    Provides routine social services as a member of a multi-disciplinary team, including obtaining social histories, providing counseling and guidance, referrals to agencies that respond to the psycho-social needs of patients/clients and families. As appropriate identifies and refers, or seeks supervision on complex cases. Reports to Director of department or program, or designee.

    Qualifications

    • Bachelor's Degree in a human services.
    • Experience working in related human service setting with similar age-group and/or substantive area is preferred.
    • Demonstrated problem solving skills.
    • Demonstrated communication skills, both written and verbal.
