    This position is located in our Bonita Spring

    This position is responsible for:

    Case Worker monitors assigned caseload of clients participating in short-term life management; established program plan/goals and evaluates client's progress by conducting routine meetings with client and/or staff.

    Key responsibilities:

    • Interviews applicants based on established guidelines and requirements; conduct needs assessment, obtain pertinent information, and establish program goals for eligible clients.
    • Develops intake and case plans for all assigned clients; instructs clients in completing necessary paperwork including goals and/or budget forms.
    • Meet regularly with clients to discuss and evaluate their progress; prepare accurate and up-to-date records documenting the same; receives incident reports and enforces disciplinary actions for infractions of the house guidelines.

    EDUCATION AND EXPERIENCE:

    • Two years of college coursework from and accredited college or university in Social Work, Behavioral Science, or a related field, AND
    • Two years experience working in a social or public service environment with experience assisting the public OR
    • Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
    • Ability to read, write, and communicate the English and Spanish languages.

    Licenses and Certifications:

    Valid State Drivers License
