Hope for Youth is looking for a dedicated and dynamic Case Planner to join our Nassau County PINS Diversion Program!About the PINS Diversion Program:Our Person in Need of Supervision (PINS) Diversion Program aims to prevent court involvement and out-of-home placements for youth under 18 by providing short-term, family-centered, in-home support throughout Nassau County. We focus on building problem-solving skills and linking families to long-term community resources.Your Role as a Case Planner:As a Case Planner, you will support youth and families by improving family dynamics, encouraging behavioral change, and reducing the risk of placement outside the home. This role requires flexible hours, including evenings and weekends, and a field-based presence using your own vehicle.Key Responsibilities:Carry a caseload of 15–25 familiesConduct weekly in-home visits with youth and familiesDeliver life coaching, parent skills training, and case management servicesAttend school meetings, court appearances, and service planning sessionsComplete and update CANS-NY assessmentsMaintain accurate, timely documentation and progress reportsParticipate in team meetings, supervision, and ongoing staff developmentAdhere to HFY and regulatory standards and practicesQualifications:BSW or Bachelor's in a related field with 2+ years of casework experience, or MSWStrong written, verbal, and tech skillsBilingual (Spanish/English) preferredValid NYS Driver's License & access to reliable transportation with mileage reimbursement availableAbility to work a flexible schedule to accommodate familiesPhysically able to perform duties (e.g., drive own vehicle, climb stairs, prolonged periods of walking/sitting)What We Offer:A mission-driven team dedicated to helping Long Island familiesOngoing training and professional developmentHybrid work model and supportive supervisionEligible for comprehensive benefits, including medical, dental, vision, and retirement plansExcellent training and leadership opportunities available throughout employment and more.Apply NowIf you're ready to make a difference in the lives of youth and their families, we'd love to hear from you!Please include a resume, cover letter and salary requirements.
