Responsibilities
- Coordinate and complete MDS assessments in accordance with federal and state regulations
- Ensure accurate, timely, and compliant clinical documentation
- Collaborate with interdisciplinary team members on resident assessments and care planning
- Monitor quality measures, clinical outcomes, and reimbursement opportunities
- Review clinical records to ensure documentation supports resident care and reimbursement requirements
- Support Medicare, Medicaid, PDPM, and payer-related documentation processes
- Assist with survey readiness, regulatory compliance, and quality improvement initiatives
Qualifications
- Current Registered Nurse (RN) license in the state of practice
- Strong clinical assessment, documentation, and communication skills
- Knowledge of MDS, RAI process, PDPM, and reimbursement methodologies
- Ability to manage multiple priorities and meet deadlines
Preferred Qualifications
- Experience as an MDS Coordinator, MDS Nurse, Case Mix Coordinator, RAI Coordinator, or Clinical Reimbursement Coordinator
- Experience in Skilled Nursing, Rehabilitation, Long-Term Care, or Post-Acute Care
- Knowledge of Medicare, Medicaid, PDPM, quality measures, and reimbursement systems
- Experience with Electronic Medical Records (EMR) and MDS software systems
- RAC-CT certification preferred
