Pay Range:

  • Compensation:
    • Minimum: $31.94/hour
    • Mid: $39.93/hour (10+ years of experience)
    • Maximum: $43.92/hour (15+ years of experience)
  • Shift Differentials:
    • Evening: +$2.50/hour
    • Weekend: +$2.00/hour
    • Differentials stack; evening weekend shifts receive an additional $4.50/hour.

Key Responsibilities:

  • Days: 8:00 AM – 4:30 PM, with 1 weekend every 4 weeks
  • Evenings: 12:00 PM – 8:30 PM with rotating weekends
  • Alternative Evening Schedule: 11:00 AM – 7:30 PM, Monday–Friday; weekday-only roles may rotate weekends as needed
  • Conduct comprehensive assessments of patients' medical, psychosocial, and discharge needs.
  • Develop, coordinate, and document individualized case management plans of care.
  • Collaborate with interdisciplinary teams to manage patient care, level of care, and resource utilization.
  • Conduct interdisciplinary team meetings and coordinate appropriate interventions.
  • Evaluate admissions and continued stays for medical necessity using approved criteria.
  • Assess observation patients for appropriateness of observation status.
  • Perform utilization management reviews and communicate with third-party payors.
  • Coordinate discharge planning and identify barriers to patient throughput.
  • Facilitate appropriate referrals to post-acute services, community resources, and payer case management programs.
  • Serve as a liaison between physicians, patients/families, hospital staff, payors, and outside agencies.
  • Monitor quality, compliance, patient safety, and regulatory requirements.
  • Track and trend barriers and variances to care and recommend process improvements.
  • Identify high-risk patients and coordinate appropriate interventions and referrals.
  • Support patient education, assistance programs, and community-based resources.
  • Promote cost-effective care without compromising patient safety or quality.
  • Maintain accurate documentation of case management interventions and recommendations.
  • Participate in performance improvement initiatives and mandatory education.
  • Report patient safety concerns and significant medical errors according to established procedures.
  • Perform other duties as assigned.

Key Requirements and Technology Experience: 

  • Minimum 3 years of recent Case Manager experience in an acute care setting, with experience within the last year.
  • Candidates with 3 years of current experience in Med/Surg, Telemetry, Neuro, ICU, PCU, or ED may also be considered.
  • Candidates with Case Management experience in Home Health or Insurance may be considered if they have:
  • 3 years of total acute care experience, and
  • At least 1 year of acute care experience within the last 5 years.
  • Associate Degree in Nursing (ADN) or Nursing Diploma required.
  • Current Florida RN license or appropriate compact RN license required.
  • If holding a compact license, an active Florida RN license must be obtained within 90 days of hire.
  • Advanced Practice Registered Nurse (APRN) license may be accepted if current and compliant.
