Bond Community Health Center is seeking a compassionate, organized, and patient-focused Case Manager to join our team. The Case Manager will play an important role in helping patients navigate healthcare and community resources while addressing barriers that may affect their ability to access care and achieve positive health outcomes.

This position works closely with patients, providers, care team members, community organizations, and other partners to coordinate services, facilitate referrals, provide follow-up, and connect patients with appropriate resources.

Position Summary

The Case Manager is responsible for coordinating and supporting services for patients who may require assistance navigating healthcare, social, behavioral health, and community resources. This position works collaboratively with patients, healthcare providers, care teams, community agencies, and other stakeholders to identify patient needs, develop individualized service plans, coordinate referrals, monitor progress, and promote improved health outcomes.

The Case Manager serves as an advocate and resource for patients while helping reduce barriers to care and connecting individuals and families with appropriate medical, behavioral health, social service, financial, housing, transportation, and other community resources.

Requirements

Essential Duties and Responsibilities

• Conduct comprehensive assessments to identify patient needs, barriers to care, available resources, and areas requiring intervention.
• Develop individualized case management and service plans in collaboration with patients and appropriate members of the healthcare team.
• Coordinate patient services across medical, behavioral health, social service, and community-based providers.
• Assist patients with navigating the healthcare system and accessing appropriate services.
• Make referrals to internal programs and external community resources and follow up to determine whether services were received.
• Maintain regular contact with assigned patients to monitor progress and identify changing needs.
• Advocate for patients experiencing barriers related to healthcare access, transportation, housing, food insecurity, insurance, financial assistance, or other social determinants of health.
• Assist patients with applications and documentation for eligible programs and community resources.
• Collaborate with providers, nurses, behavioral health professionals, social workers, and other members of the interdisciplinary care team.
• Participate in care coordination meetings and case conferences as appropriate.
• Monitor referrals, appointments, treatment plans, and other identified patient needs.
• Document patient contacts, assessments, referrals, interventions, and outcomes accurately and timely in the electronic health record and other required systems.
• Maintain confidentiality and protect patient information in accordance with HIPAA and applicable federal and state requirements.
• Identify patients who may benefit from additional services and communicate concerns to the appropriate clinical or care management personnel.
• Assist with follow-up activities for patients who have missed appointments or require additional assistance maintaining continuity of care.
• Develop and maintain knowledge of local, state, and federal health and social service resources.
• Establish and maintain effective working relationships with community organizations and service providers.
• Participate in outreach, health education, community events, and other patient engagement activities as assigned.
• Assist with quality improvement initiatives designed to improve patient access, care coordination, patient engagement, and health outcomes.
• Maintain accurate and organized case management records and reports.
• Attend staff meetings, training sessions, and professional development activities.
• Follow Bond Community Health Center policies, procedures, safety standards, and applicable regulatory requirements.
• Perform other duties as assigned.

Required Qualifications

Education:

• Minimum of an Associate Degree from an accredited college or university in Social Work, Human Services, Psychology, Sociology, Healthcare Administration, Public Health, Behavioral Health, or a related field.

Experience:

• Minimum of two (2) years of relevant professional experience in case management, care coordination, healthcare, social services, behavioral health, community services, patient advocacy, or a related field.

Preferred Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Public Health, Healthcare Administration, or related field.
• Experience working in a community health center, federally qualified health center (FQHC), healthcare, behavioral health, or social services environment.
• Experience working with underserved or medically vulnerable populations.
• Experience with electronic health records (EHRs).
• Experience coordinating healthcare and community-based resources.
• Knowledge of local community resources and social service programs.
• Bilingual or multilingual skills are a plus.

• Basic proficiency with Microsoft Office and electronic health record systems.

Work Environment & Physical Requirements

The position requires the ability to sit, stand, walk, communicate effectively, use a computer and other office equipment, and occasionally lift or move light office materials.
