Compensation: In accordance with California law, the expected salary range for this position is posted with this position. The actual compensation will be determined based on experience and other factors permitted by law.
About the Program:
Permanent Supportive Housing Program (Beverly Terrace): This low-income housing program serves homeless and chronically homeless families, as well as families at-risk for homelessness.
Role Summary: Working under the supervision of the Program Manager, the Case Manager will be responsible for providing intensive case management services (ICMS) in a residential setting to formerly homeless and chronically homeless families. The Case Manager will perform specialized tasks including intensive case management services such as intake, assessment, goal setting, monitoring and reassessment, life skills, counseling, individual benefit assistance, referrals, and linkages to all tenants. The CM- will document all tenant interactions and input that data into the Homeless Management Information System (HMIS) database and (CHAMP) database. The case manager shall serve as the central point of contact and coordinator of critical services access for each of the tenants assigned to their caseload.
Schedules:
Monday through Friday, 9:00am to 6:30pm, with a flex Friday(9/80 schedule)
Monday through Friday, 8:30am to 6:00pm, with a flex Friday(9/80 schedule)

Duties and Responsibilities:
Implement the Housing for Health Intensive Case Management Services (ICMS) Program Guide and apply the "Whatever It Take" philosophy to your work as an ICMS case manager.
Ensure that clients are referred t and linked to needed services (i.e. medical care, mental health, substance abuse, food banks, transportation)
Identify resources that a tenant needs to successfully navigate the housing process
Develop, assist and facilitate weekly Life Skills and/or Informational groups that will provide tenants with support in thriving in the various Life Domains as well as their ability to maintain their housing.
Provide and maintain weekly updates in CHAMP and Clarity (HMIS) by entering case notes and permanent housing updates
Process housing applications and provide appropriate client outreach and engagement to achieve their housing and service goals.
Qualifications
Bachelor's Degree in Behavioral Science and a minimum of one year of full-time experience working with homeless individuals or families, or equivalent work experience with this population
OR
High School Diploma with a minimum of two years of full-time experience working with homeless individuals or families, or equivalent experience with this population
Must have experience working with participants with mental illness, chronic health issues, and substance use disorders.
Bilingual in Spanish/English preferred.
Must possess and maintain a valid California Driver's License and a driving record acceptable to the agency's insurance carrier.
