JOB SUMMARY: The Case Management Supervisor (reporting to the Director of Social Services) has responsibility for the following: 

ESSENTIAL DUTIES AND RESPONSIBILITIES:  

  • Directly supervise the Case Managers 

  • Provide orientation and initial training to new Case Managers, Social Work interns and other social services staff as assigned. 

  • Review daily case notes in CARES and ensure that Case Managers input case notes within 24 hours 

  • Monitor incident reports and ensure that follow up case notes are being attached 

  • Approve documentation in all charts 

  • Identify and coordinate the training and educational needs of the Case Management staff 

  • Facilitate groups that will address the needs of the clients 

  • Provide support to clients who are struggling to follow up on their ILPs 

  • Cover caseloads when necessary 

  • Document client engagement in CARES 

  • Audit the clients’ charts to ensure compliance 

  • Participate in case conferences, team meetings, staff training, community meetings, and staff    meetings 

  • Approve staff time cards 

  • Conduct employee evaluation  

  • Perform other duties as assigned. 

JOB QUALIFICATIONS: 

Educational Requirements:  

  • Master’s Degree in Social Work (Licensed Master Social Worker) or other related fields 

Experience: 

  • Minimum of 3 years of clinical experience  

  • Experience working with the mentally ill and/or homeless population  

  • Flexible and comfortable in nontraditional setting    

  • Excellent oral and written communication skills 

  • Proven supervisory skills (in a supervisory capacity for a minimum of 3 years) 

Pay Range: $64,000 - $67,000
