Dismas House of St. Louis is currently looking for Case Managers!

Job Description:

Case Manager responsibilities include, but are not limited to the following:

  • Conducting a thorough evaluation of our clients’ needs and situation to arrange a plan that is best suited for them.
  • Develop and implement individual program plans for clients.
  • Advise the clients for needed services and benefits.
  • Coordinate program plans with professional and referred staff members and communicate information to them as needed.
  • Collect supplemental information, as needed, for clients and maintain their files in an organized, compliant, and well documented fashion.
  • Assist clients in finding and maintaining employment.
  • Provide educational opportunities and resources to help clients cope with day-to-day situations and achieve their personal goals.
  • Supervise program compliance of assigned clients.
  • Prepare necessary written reports and evaluations within the required time limit.
  • Communicate and work together with other team members to help our clients get the guidance needed.
  • Performing other duties as directed by the Associate Program Director.

Job Requirements:

    Education/Experience:

  • Bachelor's Degree or Military Service in Criminal Justice, Social Work, or a field relevant to the position 
  • In addition to or in lieu of a formal degree, a minimum of 2+ years of experience in Case Management or relevant field is required.
