    HOW YOU WILL MAKE AN IMPACT:All LCSNW team members will contribute to our mission of partnering with individuals, families and communities for health, justice and hope. The Rural Health Transformation Program (RHTP) Case Manager provides intensive, person-centered case management services to families referred to through Safe Families for Children who are experiencing barriers to healthcare access, housing instability, financial hardship, behavioral health challenges, or other social drivers of health. Working within the Interventions for Safe Families program at Lutheran Community Services Northwest, this position partners with healthcare providers, community organizations, behavioral health providers, housing agencies, and other community resources to improve health outcomes by addressing social drivers of health and increasing access to coordinated care.The Case Manager works collaboratively with Safe Families staff, healthcare partners, and participating families to develop individualized service plans, coordinate healthcare and community-based services, strengthen family stability, and reduce barriers that negatively impact overall health and well-being. Through trauma-informed, strengths-based, and culturally responsive practices, the Case Manager empowers families to build resilience, increase self-sufficiency, and improve their long-term health and wellness while meeting all Oregon Health Authority Rural Health Transformation Program requirements.

    Complete intake, eligibility verification, and enrollment documentation

    Complete Family Assessment and develop individualized care plans with measurable goals

    Maintain regular contact with clients through office visits, home visits, and community-based meetings.

    Utilize motivational interviewing, trauma-informed care, and strengths-based practices.

    Coordinate services with: Primary Care Providers; Behavioral Health Providers; Hospitals and Clinics; Oregon Health Authority partners; Community-Based Organizations; Housing Providers; Employment Services; Public Assistance Programs

    Assist participants in navigating complex healthcare and social service systems and advocate for participant needs while encouraging self-sufficiency

    Monitor progress toward established goals and revise care plans as needed

    Assist participants in accessing: Medical care; Behavioral health treatment; Substance use recovery services; Housing stabilization resources; Transportation; Food assistance; Utility assistance; Financial resources; Insurance navigation; Medication assistance; Community support services

    Develop and maintain collaborative relationships with healthcare providers and community partners

    Represent Lutheran Community Services NW at community meetings and collaborative initiatives and build partnerships that improve healthcare access in rural communities

    HOW YOU WILL BE A GREAT FIT:

    Bachelor's degree in social work, psychology or counseling; or at least two years of experience working with high needs families within the last five years. Experience may be substituted for education.

    Must have well-developed communication skills, both written and verbal

    Must be neat, organized, and accurate. Must be able to multitask and set priorities

    Must be able to work well under pressure and be comfortable in a multicultural setting

    Must be able to maintain confidentiality

    Have respectful, open, unbiased, and accepting attitude toward clients and their familiesEffective at working to support education, substance use disorder treatment, behavioral health, and other presenting concernsStandard LCSNW criminal clearance, including national sex offender. Motor vehicle records, valid driver's license and auto insurance are required.
