HOW YOU WILL MAKE AN IMPACT:
All LCSNW team members will contribute to our mission of partnering with individuals, families and communities for health, justice and hope. The Rural Health Transformation Program (RHTP) Case Manager provides intensive, person-centered case management services to families referred to through Safe Families for Children who are experiencing barriers to healthcare access, housing instability, financial hardship, behavioral health challenges, or other social drivers of health. Working within the Interventions for Safe Families program at Lutheran Community Services Northwest, this position partners with healthcare providers, community organizations, behavioral health providers, housing agencies, and other community resources to improve health outcomes by addressing social drivers of health and increasing access to coordinated care.
The Case Manager works collaboratively with Safe Families staff, healthcare partners, and participating families to develop individualized service plans, coordinate healthcare and community-based services, strengthen family stability, and reduce barriers that negatively impact overall health and well-being. Through trauma-informed, strengths-based, and culturally responsive practices, the Case Manager empowers families to build resilience, increase self-sufficiency, and improve their long-term health and wellness while meeting all Oregon Health Authority Rural Health Transformation Program requirements.
