A Case Manager RN opportunity is available in Tallahassee, FL at a 288-bed acute-care hospital offering 24/7 emergency services, a Behavioral Health Center, Neonatal Intensive Care Unit, Stroke Center, Heart & Vascular Center, and a full range of surgical and specialty services.
The role. Reporting within the interdisciplinary care team, the RN Case Manager serves as the central coordinator for evaluating clinical appropriateness, medical necessity, admission status, level of care, and resource utilization across the episode of care. The role involves conducting and documenting comprehensive psychosocial and medical assessments, developing individualized case management plans of care, and leading interdisciplinary team meetings to ensure all clinical disciplines collaborate effectively on patient goals. The RN Case Manager performs utilization management reviews and communicates findings to third-party payors, evaluates observation patients for status appropriateness, and escalates medical necessity or admission status concerns through the established chain of command. This position also identifies and addresses barriers to throughput and quality outcomes, facilitates discharge planning, and connects patients — including those with chronic conditions or limited resources — to appropriate community-based programs and post-discharge services.

Compensation. $31.94 - $43.92/hr plus $10,000 sign-on bonus
