    Available Shift

    • Evening Shift: 12:00 PM 8:30 PM EST

    • Schedule: 1 weekend per 4-week cycle

    Job Summary

    We are seeking an experienced Case Manager RN to join a 288-bed acute care setting serving Leon and Gadsden Counties. This facility offers a wide range of services including Emergency Care, Behavioral Health, Cancer Services, Heart & Vascular Care, Orthopedics, NICU, Rehabilitation, Surgical Services, Stroke Care, Imaging, and more.

    The RN Care Coordinator is responsible for overseeing the interdisciplinary plan of care, ensuring medical necessity, appropriate resource utilization, and timely patient throughput. This role is focused on achieving quality outcomes while coordinating safe, efficient, and effective discharge planning.

    Key Responsibilities

    • Perform comprehensive assessments of medical and psychosocial needs

    • Develop, document, and coordinate individualized case management plans

    • Lead interdisciplinary collaboration to ensure appropriate level of care

    • Conduct medical necessity reviews using approved criteria

    • Evaluate admission status and observation appropriateness

    • Perform utilization management reviews and communicate with payors

    • Act as liaison between physicians, families, care teams, and external agencies

    • Ensure compliance with regulatory standards, ethics, and quality initiatives

    • Identify barriers to care and develop action plans for improvement

    • Coordinate discharge needs and post-acute services

    • Support under-resourced patient populations with education and community resources

    • Provide guidance for high-risk patients and chronic disease management

    • Track, trend, and resolve variances in care

    • Participate in performance improvement, patient safety, and reporting processes

    • Maintain punctual, dependable attendance

    • Perform additional duties as assigned

    Qualifications

    • Minimum 3 years of recent Case Management experience in an acute care setting (within the last year)

    • OR 3 years of experience in Med/Surg, Telemetry, Neuro, ICU, PCU, or ED

    • Candidates with Home Health or Insurance Case Management experience considered if they have:

      • 3 years of total acute care experience

      • At least 1 year of acute care experience within the last 5 years

    • Associates Degree or Diploma in Nursing required

    • BSN preferred

    • Current Florida RN license or compact license

      • Compact holders must obtain Florida license within 90 days

    • APRN license acceptable if current and compliant

    • Case Management or Utilization Review certification preferred

    Reporting Structure

    • Reports to the Director of Case Management

    Compensation

    • Hourly Range:

      • $31.94/hr -

         $43.92/hr
    • Shift Differentials:

      • Differentials stack for up to $4.50/hr
