Job Summary:
Hiring an experienced RN Case Manager to oversee care coordination, utilization review, discharge planning, and patient advocacy while collaborating with the interdisciplinary care team.
Requirements:
- Active FL RN License or Compact License
- Associate Degree in Nursing (BSN preferred)
- 3+ years of recent Case Management experience in Acute Care
- Will also consider candidates with Med/Surg, Tele, Neuro, ICU, PCU, ED, Home Health, or Insurance Case Management backgrounds (meeting experience requirements).
Compensations:
- Pay: $31.94 – $43.92/hr (based on experience)
- Shift Differential: Evening $2.50/hr | Weekend $2.00/hr
- Sign-On Bonus: Case-by-case
- Relocation Assistance: Case-by-case
