A 221-bed acute care hospital and Level II Trauma Center in Sanford is seeking a Case Manager Registered Nurse to support inpatient care coordination across all hospital units. This facility offers a comprehensive range of services, including advanced cardiovascular care, orthopedic and spine programs, robotic surgery, inpatient rehabilitation, and full diagnostic imaging capabilities.

Position Overview

This role is centered on supporting patients and families throughout the inpatient care experience, ensuring smooth transitions from admission through discharge. The Case Manager RN works closely with physicians, nurses, therapists, and administrative leaders to coordinate care plans that are individualized, goal-oriented, and aligned with each patient’s needs.

A strong emphasis is placed on communication, discharge planning, psychosocial assessment, and collaboration with the interdisciplinary team to promote safe and effective transitions across the continuum of care.

Key Responsibilities

• Conduct psychosocial and discharge planning assessments for hospitalized patients
• Develop and implement individualized care plans based on patient needs and recovery goals
• Coordinate with physicians, nurses, and ancillary services to support continuity of care
• Facilitate communication between patients, families, caregivers, and the care team
• Educate patients and families to support engagement in care planning and recovery goals
• Document assessments, care plans, and discharge plans in a timely and accurate manner
• Collaborate with rehabilitation and case management leadership to support patient flow
• Identify barriers to discharge and work to resolve delays in care transitions
• Participate in interdisciplinary meetings and care conferences

Qualifications

• Minimum of 2 years of recent acute care Case Management RN experience within the last year required
• Current Registered Nurse (RN) license or Advanced Practice Registered Nurse (APRN) credential required
• Associate degree in Nursing or Nursing diploma required
• Strong understanding of acute care workflows and discharge planning processes
• Excellent communication, assessment, and coordination skills in a hospital setting
