We are seeking an experienced Case Manager RN to join a leading acute care hospital in Sanford, Florida.
Schedule
- Full-Time
- Day Shift
- Monday–Friday
- Rotating weekends (every other Saturday, possibly every 3rd Saturday once fully staffed)
Compensation
- $36.96 – $44.00/hr (based on experience)
- Weekend Differential: $1.50/hr
- Sign-On Bonus: Case-by-case
- Relocation Assistance: Case-by-case
Responsibilities
- Coordinate patient care and discharge planning
- Conduct psychosocial and discharge assessments
- Develop individualized care plans
- Educate patients, families, and caregivers
- Collaborate with physicians, nurses, and interdisciplinary teams
- Ensure smooth transitions of care and resource coordination
Requirements
- Active RN License
- Associate Degree in Nursing or RN Diploma
- Minimum 2 years of recent Acute Care Case Management RN experience (within the last year)
- Strong communication and care coordination skills
