    Registered Nurse Case Manager – Direct Hire

    Location: Nanticoke, PA Schedule: Monday–Friday, 8:00 AM–6:00 PM Additional Requirements: 1 weekend per month + 2 holidays per year Travel: Local travel required across multiple counties

    Overview

    This is an excellent opportunity for a compassionate and experienced Registered Nurse Case Manager to join a supportive hospice team in Nanticoke, PA. In this role, you will provide essential care coordination, leadership, and patient advocacy for individuals receiving hospice services. You'll work closely with interdisciplinary teams to ensure patients and families receive exceptional end‑of‑life care.

    Key Responsibilities

    • Manage and oversee patient care within hospice settings
    • Conduct comprehensive assessments and develop individualized care plans
    • Coordinate care with physicians, social workers, and interdisciplinary teams
    • Provide leadership, mentorship, and support to nursing staff
    • Ensure compliance with hospice standards, policies, and regulatory requirements
    • Maintain accurate documentation and timely reporting
    • Travel locally to visit patients across assigned counties

    Required Qualifications

    Education:

    • High School Diploma or GED

    Experience:

    • 1+ year Hospice experience
    • 1+ year Management or Leadership experience
    • 1+ year experience in ANY of the following:

      • Home Health Care
      • Epic Home Health
      • Home Health

    Licensure:

    • Valid Registered Nurse (RN) license in the state of Pennsylvania (PA)

    Certifications (ALL required):

    • American Heart Association Basic Life Support (AHA BLS)
    • American Heart Association Advanced Cardiac Life Support (AHA ACLS)

    Schedule & Shift Details

    • Monday–Friday, 8:00 AM–6:00 PM
    • On‑Call:

      • One weekend per month
      • Two holidays per year (1 summer, 1 winter)

    Travel Requirements

    • Local travel required across multiple counties
    • Must be comfortable working in field‑based hospice environments
