Description
A New York City-based health services organization is currently seeking a licensed Registered Nurse (RN) to join their team in the Bronx as their new Case Manager. This role focuses on clinical review, care coordination, and multidisciplinary collaboration in a hospital setting.
About the Opportunity:
- Shift: Days
- Schedule: Monday to Friday
- Hours: 8:30am to 4:30pm or 9am to 5pm
- Setting: Acute Care
Responsibilities:
- Conduct telephonic clinical reviews for behavioral health patients
- Complete PRI and Screen assessments
- Coordinate behavioral health patient care and case management activities
- Manage behavioral health caseloads
- Participate in interdisciplinary team meetings
Qualifications:
- Minimum 2 years of recent Behavioral Health experience
- Bachelor's Degree in Nursing
- Active NYS Registered Nurse (RN) license
- AHA BLS certification
- PRI and Screen certification
- Case Management background
Desired Skills:
- Care management certification
- Experience with EPIC EMR
- Telephonic Clinical Review experience
- Experience in a Leadership role
