Case Manager RN - Full Time | Sign-on & Benefits

We are seeking an experienced Case Manager RN to join a collaborative healthcare team dedicated to delivering high-quality, patient-centered care. The ideal candidate will have strong acute care experience, excellent communication skills, and the ability to coordinate care plans, discharge planning, and patient advocacy across the continuum of care. This is an excellent opportunity for a motivated RN looking to grow within a respected healthcare organization.

Shift: Days | 8:00 AM – 4:30 PM | Rotating Weekends

Qualifications needed:

• Minimum 2 years of recent acute care experience within the last 3 years
• Preferred backgrounds include ICU, CCU, Med-Surg, LTAC, or Case Management experience
• Active RN License
• Associate degree or Nursing Certificate required
• Strong communication, critical thinking, organizational, and advocacy skills required
• Candidates should be confident, coachable, adaptable, and comfortable collaborating with physicians regarding safe discharge planning

Compensation & Benefits:

• Salary Range: up to $47.10/hr (based on years of experience)
• $10,000 Sign-On Bonus
• Relocation Assistance available on a case-by-case basis
