A 267-bed acute care hospital in Fort Walton Beach is seeking a Case Manager Registered Nurse to support patient care coordination across the continuum of care. The facility offers a wide range of services, including an Advanced Primary Stroke Center, Cancer Care Center, open-heart surgery, a full-service emergency department, pediatric emergency care, and freestanding emergency departments.
Position Overview
This role focuses on guiding patients and families through the full care journey, ensuring smooth transitions and appropriate use of healthcare resources. The Case Manager RN works closely with patients, families, physicians, therapists, and administrative teams to develop individualized care plans that support recovery goals and long-term outcomes.
A strong emphasis is placed on psychosocial assessment, discharge planning, education, and interdisciplinary collaboration to ensure safe and effective transitions from admission through discharge.
Key Responsibilities
• Coordinate patient care across multiple disciplines and service lines
• Conduct psychosocial assessments and discharge planning evaluations
• Develop individualized care plans aligned with patient needs and recovery goals
• Educate patients, families, and caregivers on treatment plans, benefits, and discharge expectations
• Provide program orientation covering rehabilitation philosophy, insurance coverage, and patient rights
• Document clinical findings, care plans, and discharge plans accurately and timely
• Facilitate communication between patients, families, and the interdisciplinary care team
• Collaborate with rehab leadership and case management leadership to support continuity of care
• Support safe and timely transitions across the care continuum
• Identify barriers to discharge and assist in resolving care coordination challenges
Qualifications
• Associate degree or nursing certificate required
• Active Registered Nurse (RN) license required
• Minimum of 2 years of recent acute care experience within the past 3 years required
• ICU, CCU, Med-Surg, or LTAC case management experience preferred
• Strong communication, assessment, and care coordination skills required
Schedule
• Full-time, day shift
• 8:00 AM to 4:30 PM
• Rotating weekends required
